Short Description of Instrument |
The Non-motor Symptoms Questionnaire (NMS-Quest) (PD-NMS) is a self-completed patient questionnaire with 30 qualitative questions designed to identify the presence but not the severity of non-motor symptoms.

Scoring and Psychometric Properties |
Scoring: Each of the 30 questions presents a problem that patients may have experienced within the past month. The patients may give an answer of "yes", "no", or "don't know" to each of the 30 questions.
Suggested scoring scheme: 0 = No NMS; 1-5 = Mild; 6-9 = Moderate; 10-13 = Severe; and >13 = Very severe
Psychometric Properties: Low ceiling and floor effects; strong correlation with H&Y stage; 75% of PD caregivers and patients felt NMSQuest helped doctors treat PD better, while over 90% reported that the items contained in the NMSQuest were appropriate and relevant to day-to-day life.

Rationale/Justification |
Strengths: PD-specific scale, brief administration time (5-7 minutes), self-administered, validated in multiple languages, and initially developed by an international multicenter collaboration.
Weaknesses: Developed as a screening questionnaire rather than an outcome measure for severity or progression of disease.
